Registration Information Conference Hotel Information Crowne Plaza Riverwalk Hotel and Conference Center Our host hotel is the Crowne Plaza Riverwalk Hotel and Conference Center, 111 Pecan Street East. Located on the River Walk at Soledad Plaza in downtown San Antonio, the hotel is within walking distance of the Alamo, Rivercenter Mall, and Hemisfair Plaza. A special room rate of $125 (plus tax) single/double, is guaranteed for reservations made by June 3, 2008 or until the room block is filled. Reservation requests after this date will be accepted on a space-available and rate-available basis. Make your reservation today by calling the number above. Please identify yourself as a participant in the 2008 Summer Institute on Evidence-Based Practice. Book early to assure a room at the hotel. Registration Information Please complete the registration form and submit to the Department of Continuing Dental Education. You can register:
*Registrations must be postmarked or faxed by May 14, 2008 to qualify for the Early Registration Fees or by June 11, 2008 to qualify for the regular fees. Registrations postmarked after June 11, 2008 will be accepted at onsite rates (call (210)-567-3177 to find out if space is available). Purchase Orders will not be accepted. Be sure to indicate your preference for the concurrent sessions you wish to attend. This will help us to provide appropriate seating during the Institute. Registration Confirmation Written confirmation will be sent by mail or fax if your registration is postmarked by June 11, 2008. For those that registered on or before June 11, 2008 and you did not receive a confirmation, please email inquiries to smile@uthscsa.edu. If your registration was not postmarked by June 11, 2008, contact (210) 567-3177 to inquire about space availability for the conference. You are responsible for ensuring that your registration has been received. Refunds & Cancellation Policy UTHSCSA Academic Center for Evidence-Based Practice reserves the right to limit enrollment or to cancel any course no less than one week prior to the activity. Should circumstances make this necessary, only registered attendees will be informed. In this event, tuition would be refunded in full. You are encouraged to send a substitute if you are unable to attend, once registered. Cancellations must be received in writing and postmarked or faxed no later than June 11, 2008. Cancellations cannot be accepted over the telephone. The registration fee will be refunded, minus a $100 administrative fee if notice is received by June 11, 2008. After June 11, 2008 there will be no refund of registration fees. The conference hosts reserve the right to substitute speakers or cancel and reschedule sessions due to any unforeseen circumstances.
