Registration Information Conference Hotel Information Adams Mark Hotel Our host hotel is the Adams Mark Hotel, 111 Pecan Street East. Located on the River Walk at Soledad Plaza in downtown San Antonio, the hotel is within walking distance of the Alamo, Convention Center, and Hemisfair Plaza. A special room rate of $105 single/double is guaranteed for reservations made by June 20, 2003 or until the room block is filled. Reservation requests after this date will be accepted on a space-available and rate-available basis. Make your reservation today by calling the number above. Please identify yourself as a participant in the 2003 Summer Institute on Evidence-Based Practice. Book early to assure a room at the hotel. The University of Texas Health Science Center at San Antonio Be sure to indicate your preference for the concurrent sessions you wish to attend. This will help us provide appropriate seating during the Institute. Registration Confirmation Written confirmation will be sent by email (preferred), fax or mail, if your registration is received by June 25, 2003. If you are registering less than 2 weeks before the program, or if you do not receive a confirmation letter, call the Nursing Continuing Education office at The University of Texas Health Science Center at San Antonio (210)567-5850 to check on space availability. You are responsible for ensuring that your registration has been received. Registration Cut-Off Date Mail registration must be postmarked no later than June 25, 2003 for registration. The cut-off date for registration is July 7, 2003. After that date, you must call to confirm space availability and receive priority for on-site registration if space is available. Refunds, Cancellation Policy You are encouraged to send a substitute if you are unable to attend, once registered. The cancellation must be received in writing, post marked no later than June 25, 2003. Cancellations cannot be accepted over the phone. The registration fee will be refunded, minus a $50.00 administrative fee. After June 25, 2003 there will be no refund of registration fees. The conference hosts reserve the right to substitute speakers or cancel and reschedule sessions due to any unforeseen circumstances. |
