Our host hotel is the Crowne Plaza Riverwalk Hotel, 111 Pecan Street East. Located on the River Walk at Soledad Plaza in downtown San Antonio, the hotel is within walking distance of the Alamo, Convention Center, and Hemisfair Plaza.

A special room rate of $108 (plus tax) single/double is guaranteed for reservations made by June 22, 2005 or until the room block is filled. Reservation requests after this date will be accepted on a space-available and rate-available basis. Make your reservation today by calling the number above. Registration Information 

Please complete the registration form in this brochure. You can register:

1.  By FAX*:  Fax the completed registration form with credit card information (VISA, MasterCard, or Discover) to Continuing Nursing Education, Fax (210) 567-5909.
2.  By MAIL*:  Send check or money order (payable to UTHSCSA-ACE) with the registration form to: The University of Texas Health Science Center at San Antonio, Continuing Nursing Education Department, 7703 Floyd Curl Drive, MSC 7946, San Antonio, TX 78229-3900.
Registrations must be postmarked or faxed by May 25, 2005 to qualify for the Early Bird Fee of $450, or by June 22, 2005 to qualify for the regular fee of $475. Registrations received after June 22, 2005 will be accepted on site for the fee of $525. Be sure to indicate your preference for the concurrent sessions you wish to attend. This will help us provide appropriate seating during the Institute.

Pre-Conference Registration
Registration for the “Teaching EBP and Evidence-Based Teaching” Workshop is $150 if received by June 22, 2005 or $175 on-site. Registration for the “Pre-Institute Session-Novice Track” is $50 if received by June 22, 2005 or $70 on-site.

Registration Confirmation

Written confirmation will be sent by e-mail or fax if your registration is received by June 22, 2005. if you are registering less than 2 weeks before the program, or if you do not receive confirmation, call the Academic Center for Evidence-Based Practice office at 210-567-1480 to confirm registration and availability.

Refunds and Cancellation Policy

UTHSCSA Academic Center for Evidence-Based Practice reserves the right to limit enrollment or to cancel any course no less than one week prior to the activity. Should circumstances make this necessary, only registered attendees will be informed. In this event, tuition would be refunded in full.

You are encouraged to send a substitute if you are unable to attend, once registered. The cancellation must be received in writing and postmarked or faxed no later than June 22, 2005. Cancellations cannot be accepted over the telephone. The registration fee will be refunded, minus a $50 administrative fee if notice is received by June 22, 2005. After June 22, 2005 there will be no refund of registration fees. The conference hosts reserve the right to substitute speakers or cancel and reschedule sessions due to any unforeseen circumstances.
